1. Insert the USB flash drive into the USB port of your computer

2. Go to My Computer and click on the Removable Drive icon.

3. Click on the Ready Med Info folder to open the form

4. The Ready Med Into form is opened as an Adobe Acrobat PDF file.

5. When the form is opened, use the mouse to point to each field and left click for typing. Also point and click the mouse to select and left click for typing. Also point and click the mouse to select/deselect the box you wish to mark in the Yes/No section.

6. The form can be completed all at once or filled out in sections depending on your available time.

7. Once you have completed the form to your satisfaction, save the information directly to the USB flash drive by locating the SAVE AS icon on your computer and left clicking the mouse to save.

8. Safely remove the USB flash drive from the computer's USB port.

9. The Ready Med Info form may be reused and updated by following the steps outlines above.

10. The USB flash drive can be taken to the doctor's office, hospital, or presented to a paramedic when needed.
